INTRAVASCULAR PAPILLARY ENDOTHELIAL HYPERPLASIA OF THE ORAL CAVITY: A RARE
            CASE REPORT

Ong YY, Sharif JM, Sockalingam G

Introduction: Intravascular papillary endothelial hyperplasia (IPEH), also known as
Masson’s tumor, is a benign lesion of the skin and mucosa of vascular origin characterized
by reactive proliferation of endothelial cells. IPEH’s etiology is unknown and comprises
approximately 2% of all vascular proliferation cases of the skin and subcutaneous tissues.
Although IPEH may occur in any blood vessel, occurrence in the oral cavity is very rare. Case
report: A 10-year-old girl was referred to our department with a present of a recurrent
swelling in the mouth at the lower anterior ridge. Previously, patient has undergone 2
excisional biopsies under general anesthesia in a local hospital at her hometown and the
histopathology reveals a pyogenic granuloma. Clinical examination showed a soft, raised,
non-pulsatile bluish red lesion, measuring about 1cm x 1cm. Due to the recurrence, patient
underwent another excisional biopsy under general anesthesia. This time, the
histopathology revealed a diagnosis of IPEH. Three months after surgical resection, the
patient is well, with no complications or evidence of recurrence. The pathogenesis of IPEH
is poorly understood. In majority of cases, there are no significant signs and symptoms or
complications associated with this condition. However, clinically it can mimic many
pathological lesions, both benign and malignant. Conclusion: It is important for clinicians
and pathologists to be aware of this lesion to avoid a misdiagnosis and prevent unnecessary
aggressive treatment. Surgical removal is the treatment of choice and the prognosis is
            generally excellent.
