DREDGING: AN ALTERNATIVE APROACH TO EXTENSIVE AMELOBLASTIC FIBROMA: A CASE

            Introduction: Ameloblastic fibroma (AF) is a rare benign odontogenic tumor of epithelial-
            mesenchymal  origin as it accounts for  only  2%  of  odontogenic tumor. Generally,
            ameloblastic fibroma is painless, slow growing and predominantly found in  children.
            Literature has shown that they are recommended to be managed with resection of the jaw
            to  lower the risk  of recurrences.  Case report:  A 7-year-old  Malay girl referred to our
            department with painless swelling  in the  left cheek region  3  months ago and  its size
            enlarged gradually. The Orthopantomogram (OPG), Cone Beam Computed Tomography
            (CBCT) and Computed Tomography (CT) Scan taken showed a large  multilocular
            radiolucency  in the  left mandible from tooth 75 region extending up to left condyle,
            complicated with displaced tooth 35,36,37 and distal root resorption of tooth 75. Incisional
            biopsy was done and the histopathological examination (HPE) result confirmed it as AF. In
            view of the extent of the lesion and patient’s age, a conservative approach was chosen as
            the principal treatment of ameloblastoma which is enucleation and dredging even though
            the chances of recurrences for a multilocular ameloblastic fibroma lesion is extremely high.
            During the 7 months of post dredging, positive improvements can be seen with formation
            of new bone for eventual achievement of normal contour of jaw and no signs of recurrence
            noted.  Conclusion:  This  present case highlights  that the Ameloblastic fibroma (AF)
            successfully being managed  by using a conservative approach such  as  enucleation,
            chemical curettage and dredging method while a radical surgery e.g resection of the jaw
            can be avoided.
