GIANT NECK LUMP: A CASE REPORT

Introduction: Neck lumps are common in the general population, presenting in both
children and adult. Neck lumps can be large and visible, or they can be very small in size
and be an incidental finding during examination. The potential differentials for neck lumps
are wide and can be classified into congenital or developmental, inflammatory and
neoplastic. Case report: A 40 years old Malay gentleman presented to Oral Maxillofacial
Surgery Department with a painless swelling on his right neck for the past four years. The
growth started as a small swelling and gradually increased in size. The lump caused
discomfort due to heaviness and tightness of the neck that disturbed his daily routine.
With the proper planning and careful surgical approach, also without compromising the
surrounding vital structures the lump was successfully removed. The lump was completely
excised, where the vessels and the nerves were preserved. The excised lesion measured
10.5 x 7.5 x 5.3 cm³ and together with a lining resembling a pouch that was attached to
lymph node level 5 was removed. Post-operatively at 1-year of follow up the patient
recovered with no obvious vessel or nerve damage and led to ward discharged and no
follow-up was given. Conclusion: Branchial cleft cysts remain a rare entity in adults. Our
case report of this 40 years old gentleman shows that branchial cleft cysts should be
considered in the differential diagnosis for patients presented with neck swelling in older
patients.
