A RARE CASE OF NEURILEMMOMA OF THE PALATE

Introduction: Neurilemmoma or Schwannoma is a slow growing, benign, encapsulated
neoplasm that arises from Schwann cells of the neural sheath of the peripheral nervous
system. Preoperative diagnosis is often difficult, although radiological imaging is helpful,
a confirmatory diagnosis can only be made by histopathological examination. Occurring
as a common tumor in the head and neck region, its intraoral presentation is rather rare,
tongue being the most common site. Case report: A 47-year-old male who was made
aware of a soft tissue lesion over his palate during a regular visit to his dentist and he was
referred to our department for further management. On intra-oral examination, patient
had a well-defined soft tissue swelling, firm in consistency with a smooth non
erythematous overlying mucosa, extending from mid palate slightly to the soft palate. A
definitive diagnosis of Neurilemmoma was obtained once an incisional biopsy was done.
Patient subsequently underwent a wide local excision of the lesion along with peripheral
ostectomy over the palate, it was noted the bony palate at its base was intact upon careful
dissection. Histopathological examination of the soft tissue was noted to be composed of
spindle cells with palisaded nuclei and prominent Verocay bodies. The tumor cells showed
strong positivity for S100 in immunohistochemistry staining. Conclusion: Neurilemmoma
should be considered as a differential diagnosis when observing soft tissue neoplasms in
the oral cavity.
