ANKYLOSED TEMPOROMANDIBULAR JOINT IN A CHILD
CASE REPORT

Case report: A child with a history of being unable to open his mouth for 6 years and
obstructive sleep was presented to our department. Patient was diagnosed with ankylosis
of the left temporomandibular joint, micrognathia and obstructive sleep apnoea which
requires multi-disciplinary approach to ensure best and safest treatment option and
sequence. To achieve Maximal Incisal Opening and relieve of narrow posterior airway
space, the release of ankylotic right Temporal-mandibular joint was done. Adequate
mouth opening was achieved in the immediate postoperative period. Intensive jaw
exercise was mandatory to achieve and maintain an acceptable Maximal Incisal Opening.
However, additional mandibular advancement techniques to further correct the
retrognathia were required to improve the post-operative Apnoea-Hypopnea Index.
Conclusion: A triad of temporomandibular joint ankylosis, micrognathia and obstructive
sleep apnoea in a young patient requires the involvement of multiple disciplines.
