MUCORMYCOSIS; MAXILLARY SINUS TO ORBIT AND VICE VERSA

Introduction: Mucormycosis is a rare condition. The presentation of this disease differs
from case to case and most common among patients with diabetes mellitus. Objective:
To highlight this unusual disease and to avoid incorrect diagnosis. Case report: A 38-year-
old Malay female diagnosed with type II diabetes mellitus presented with blurring of
vision and pain over her right eye for one week. After multiple computed tomography
imaging and fibre optic scope, proceeded with inspection surgery at right maxillary sinus.
Large amount of cheesy, soft and spongy like structures were removed from the right
maxillary sinus and sent to histopathological examination which led to the diagnosis of
Mucormycosis. Patient was immediately treated with aggressive antifungal medication.
Since then, patient had uneventful healing with the right eye full vision. Conclusion:
Fungal infection may present in diabetic patients with uncharacteristic features. For
prompt management and to safe patient’s eyesight utilising multiple methods such as
radiograph and histopathological examination was essential. Hence, delay in treatment
can be life threatening due to tendency of fungi to invade adjacent blood vessels and
embolize to distance organ including brain.
