ADENOMATOID ODONTOGENIC TUMOUR (AOT): A CASE REPORT

Case report: A 13 years and 8 months old Indian girl was referred to paediatric dentistry
clinic for further management of swelling and pain on upper left buccal segment. The
swelling was first noticed by her 7 months ago and was associated with spontaneous sharp
pain that radiating to middle facial region. Upon examination, facial asymmetry noted with
swelling extending from left nasolabial fold to left malar region. Intraorally, there was a
solitary smooth circumscribed swelling with 3cm x 3cm in size noted in relation to buccal
segment of teeth 23, 24 and 25. Mild tenderness and egg-shell crackling sound noted upon
clinical palpation. Full investigations were conducted prior to surgical enucleation of the
adenomatoid odontogenic tumour (AOT) which was performed under general anesthesia
(GA), a month after her first visit. Post-operative regular review showed her general
condition improved, wound healed well and an acrylic upper partial denture was issued.
The distinctive histopathological features in this case (i.e., the thick and fibrotic capsule
around the lesion, columnar epithelial cells form rosette /ductlike structures that are
hollow in the middle) are crucial in the diagnosis AOT. The total surgical enucleation was
performed during excisional biopsy under GA in consideration of the presence of thick and
fibrous capsulated lesion with clear demarcation intraoperatively. Conclusion: The
presented case report highlighting the exceptional aggressive nature in AOT and the
importance of the histopathologic examination in the cystic lesions of the maxillary bones
to ensure correct management of this tumour and involvement of any associated teeth.
