PRESERVATION OF SUPERFICIAL LOBE FOR DEEP-LOBE PAROTID TUMOR: A CASE
REPORT

Introduction: Deep lobe of parotid tumor is relatively uncommon. Traditionally, a total
parotidectomy technique is used to remove parotid tumor. This conventional technique
may result in multiple post-operative complications. By preserving superficial lobe that
made up approximately 80% of parotid tissue volume intraoperatively, the likelihood to
get multiple complications such as facial nerve paralysis, gustatory sweating and aesthetic
defect due to loss of volume may occur less post operatively. Therefore, we decided to
use a new technique of preserving the superficial lobe for benign salivary neoplasm case
in our premise. Case report: A case of deep-lobe parotid tumor reported in Oral &
Maxillofacial Surgery Hospital Shah Alam. A 59-year-old Malay gentleman presented with
swelling of his left parotid region over a 2-month period. It was painless, slowly growing
in size, causing discomfort but not disturbing his daily routine. CT neck scan done prior to
the surgery. Superficial lobe of parotid gland was raised together with the skin flap to
expose the deep-lobe parotid tumor. Nerves, vessels and superficial lobe of parotid gland
are preserved intraoperatively. Tumor is excised completely. Patient is still continuing his
follow-up under our care with minimal complications and is recovering well. Conclusion:
This case report highlights deep-lobe parotid tumor excision with preservation of
superficial lobe of parotid gland can be a viable option for better patient outcomes,
without compromising eradication of tumor.
