AN OPTION TO RESTORE VERTICAL DIMENSION WITH OVERDENTURE: CASE REPORT

Introduction: Extraction of teeth can lead to reduced occlusal vertical dimension resulting
in masticatory function impairment, dental aesthetics and facial appearance. Providing an
overdenture underneath remaining tooth may contribute to better retention with
periodontal receptors for proper mastication and accurate jaw movement. Case report: A
61-year-old gentleman was referred for management of reduced vertical height that has
disturbed his facial muscles movements. He felt pain on both temporal region areas
especially on the right side. He had many dentures made to relief the symptoms but yet
none has helped. Treatment was rendered to evaluate the outcome of increasing patient’s
occlusal vertical dimension with temporal pain symptom and to follow up the influence of
corrective treatment with partial overdenture and crowned posterior teeth. The vertical
dimension was initially increased with provisional crowns of posterior teeth followed by a
partial overdenture for three months. A natural tooth which was retained with a telescopic
crown gave a good reference for clinician to restore patient’s facial height. The final metal
ceramic crowns were cemented in the review visit. A cobalt-chrome overdenture was
fabricated to replace the provisional acrylic overdenture. After insertion of the partial
overdenture, pain on the temporal region resolved and the facial height improved
significantly. This method offers simple procedures without the need to use complex face
bow transfer and it preserves the bone and alveolar ridge. During the insertion stage,
patient was happy and satisfied with his new denture and facial profile. Conclusion:
Restoring facial height with symptoms can be very challenging. Thus, overdenture can be a
good treatment option as it offers simplicity, reversibility, and relatively low cost.
