Introduction: Traumatised anterior tooth often results in pulpal blood vessel injury and
loss of vitality that may lead to a characteristic dark appearance. The discoloration of
anterior teeth can give a significant aesthetic and social impact as it may result in
considerable cosmetic impairment. Treating discoloured teeth has become a great
challenge these days due to high expectation from patients. To match the shade of a single
discoloured anterior tooth by means of a prosthesis is ever more difficult. Case report: We
present a case of severely discoloured anterior tooth in an adult patient with post trauma.
This case report describes a 39-year-old female patient who presented with a chief
complaint of discolored and unpleasant sight of upper front tooth. Patient had a fall in 2007
and noticed that her front tooth changed colour overtime with craze lines noted on the
surface. In February 2017, her dentist informed her that the tooth is non-vital and required
root canal treatment, which she then completed in May 2017. However, the dark
appearance of the tooth remained unchanged. Non-vital beaching procedure was
subsequently done for the patient, which lightened the colour of the tooth, followed by
construction of an E-max crown. The main barrier in treatment of such cases includes the
degree of discoloration, post-trauma craze lines and keeping the treatment simple and
conservative. Conclusion: In the era of aesthetic dentistry, bleaching of discoloured teeth
has gained in popularity. However, due to the presence of craze lines on the tooth,
construction of an E-max crown is still deemed to be better in term of protection and final
aesthetics.
