EVALUATION ON  THE IMPLEMENTATION OF MODIFIED MOH ICDAS IN PROMOTING
            ORAL HEALTH  CLINICAL  PREVENTION  AS PART OF INCREMENTAL SCHOOL DENTAL
            SERVICE, KELANTAN

            Introduction:  Dental  caries is a known  multifactorial  disease and  has a  very high
            prevalence in Kelantan since long-ago. Modified MOH ICDAS was adapted by the Oral
            Health Programme  (OHP), Ministry  of Health,  Malaysia from the International Caries
            Detection and Assessment System and implemented since the year 2017 for 2 districts (4
            pilot schools), and expanded in 2018 to all the primary schools in Kelantan. School children
            who need dental preventive  care  would be given either  fluoride varnish application,
            fissure sealant, preventive resin restorations  or combinations during the Incremental
            Dental Service (IDC). National standard targets were set by the OHP, MOH. Materials and
            methods: This was an evaluation study based on the secondary data collected by the state
            for both pilot schools as compared to the cohort schools children between 2018 – 2020.
            Data for each schools/children/teams/districts were recorded in the  EZ-RIF format which
            was innovated and updated for the needs of this new index. Study evaluation was also
            conducted on the clinical audit findings on the students’ dental records as compared to
            the intra-oral examinations. Results: The pilot schools showed a higher detection of initial
            caries (53.8%) as compared to the other schools (29.1%). Findings from clincal reports
            showed that the dental therapists likely to be under-charted and have not been able to
            adapt to the new index whereby the dental  care were focusing  on conservative
            restorations rather than preventive treatments  by detecting initial caries. Conclusion:
            This study suggested that more training is needed for the dental therapists and further
            study should be conducted to explore their barriers in implementing the new index.
