AMELOBLASTOMA IN AN ELEVEN-YEAR-OLD CHILD: A CASE REPORT

Introduction: Ameloblastoma is a common odontogenic tumour of the maxillofacial
region; it is a locally invasive neoplasm with maximum incidence reported in the 3rd and
4th decade of life but rarely in childhood. We present a rare case of ameloblastoma in an
eleven-year-old girl. Case report: An eleven-year-old Malay girl, presented with a swelling
on the left side of her lower jaw for the past one month. The swelling had no sign of
regression or resolution. Upon examination, there was a bony hard swelling arising from
the lower left body of the mandible and intra-orally, there was a firm buccal elevation of
her mucosa extending from the lower left premolar up to lower left first molar with a
missing lower left second premolar. Radiological investigations revealed a well-defined
expansile bony thin lytic lesion of the left mandible with associated thinning of the bony
cortex. A more conservative approach of surgery was done by surgically enucleating the
lesion without a bony resection and the histopathological examination was conclusive of
ameloblastoma. Discussion: The diagnosis of ameloblastoma in children can be difficult
as it can radiographically resemble dentigerous cyst. Ameloblastoma are treated
aggressively to avoid recurrences but there is also a dilemma on the applicability of an
initial radical, extensive surgery procedure in children. Conclusion: Conservative
treatment appears to be preferable in the younger age groups because it offers a better
quality of life; however, the recurrence rate remains high constant follow-ups for the next
5 years is required.
