CONSERVATIVE MANAGEMENT OF MYOPERICYTOMA IN A VERY YOUNG CHILD: A CASE

            Introduction: Myopericytoma(MPC) is a benign neoplasm characterized by a perivascular
            proliferation of oval to spindle-shaped pericytic cells of myoid differentiation. MPC has
            similar morphological features with hemangiopericytoma, glomus tumour, myofibroma
            and solitary fibrous tumour which can be the possible differential diagnoses. Case report:
            An 11-month-old female infant presented with a slow growing bony hard swelling at the
            left ascending ramus. Incisional biopsy was reported as myofibroma. Patient was referred
            for consultation to plastic surgery and resection of left posterior mandible and
            reconstruction with rib graft was suggested but parents refused aggressive treatment.
            After two years, swelling increased in size with slight deviation of mouth opening to the
            left. As parents were only keen for a conservative approach, enucleation and curettage of
            lesion at left ramus of mandible was carried out at 3 years of age in our centre. Bismuth
            Iodine Paraffin Paste (BIPP) pack placed in cavity and maxillo-mandibular fixation (MMF)
            placed for one month as thinned lower border of mandible had fractured.
            Histopathological examination of the gross specimen was suggestive of myopericytoma.
            Clinical examination and radiographic assessment at 2 years post operation review shows
            satisfactory form and function of mandible. Since parents refused aggressive treatment
            like resection, enucleation via an intraoral approach was done to avoid facial incision,
            preserve the perioral musculature, reduce morbidity and thus giving optimal aesthetic and
            functional results. Conclusion: Although the access is fairly limited with risk of damage to
            mental nerve, this approach is relatively conservative and complications can be minimised
            with proper technique especially in children.
