Introduction: Supportive Periodontal Therapy (SPT) follows the same principles employed
in the treatment of active disease. It begins once patients are deemed periodontally stable,
which is determined six to eight weeks after completion of active periodontal treatment.
The aims of SPT are to minimize the recurrence of disease through periodic preventive
interventions and to maintain the attachment apparatus in the most stable condition
possible over the long term. However, there are still evidences showing that those patients
with well-maintained SPT still experienced tooth loss. Objective: To assess the prevalence
and reasons for tooth loss in a sample of patients who underwent active periodontal
therapy and maintained under regular SPT. Materials and Methods: A sample of 89
subjects were selected from periodontal specialist clinic between December 2014 and
December 2018. All subjects had been on recall schedule for SPT for duration of 4 years or
less. Based on the subject’s record, reasons for tooth loss were categorized as periodontal
(increased mobility, advanced furcation involvement, severe bone loss), caries, endodontal
(untreatable endodontic pathology), surgical extraction of impacted third molars and
others (tooth fracture, prosthodontic reasons, etc). Results: Almost half of the patients was
reviewed in less than 1-year (44.9%) and the mean age was 50.3 (±10.8) years. There was
a similar proportion of subjects by gender and majority was Malay (62.9%) and non-
smokers (84.3%). At the completion of active periodontal therapy, 2210 teeth were
present. During the maintenance period, only 18 teeth (0.8%) of 12 subjects were lost due
to several reasons. Periodontal problem was the most common reasons for tooth loss
(66.7%). Other reasons were due to caries and others (16.7%). Molars were found the most
frequently lost tooth, whereas canines and incisors were the least. Conclusion: The
prevalence of tooth loss in periodontal patients following maintenance periodontal
therapy was very low (0.8%) which mainly due to periodontal problem.
