PREVALENCE OF IMPACTED TEETH AND ITS ASSOCIATED CLINICAL FEATURES IN
PATIENTS ATTENDING TWO DISTRICT ORTHODONTIC CLINICS IN MALAYSIA

Objective: This cross-sectional study aimed to evaluate the prevalence, distribution,
clinical features, age at referral and treatment modality of patients with impacted teeth
attending two district orthodontic clinics, in the East coast of Malaysia. Materials and
methods: A total of 191 consecutive cases of patients who attended for orthodontic
appointment at Kemaman and Dungun from 1 January 2017 to 31 December 2018 were
selected. Patients’ clinical records, radiographs and study models were reviewed.
Descriptive statistics were performed. Results: A total of 113 patients (59.2%) had
impacted tooth, with presentation of one impacted tooth in 94 cases (49.2%), two
impacted teeth in 17 cases (8.9%) and 3 impacted teeth or more in 2 cases (1%). In the
maxilla, canines were the most commonly impacted teeth (67.8%), followed by central
incisors (13.8%) and 2nd premolars (10.3%). In the mandible, the 2nd premolars (55.1%)
were the most commonly impacted teeth followed by 2nd molars (20.4%) and canines
(16.3%). The frequency of impacted teeth was higher in the maxilla than the mandible
(ratio of 1.8 :1). Most cases were found in females (72.6%) in Kemaman district (68.1%).
The mean age of referral of patients with impacted teeth were 14 years. 46.1% of referrals
were made when the patients were aged more than 14 years. The associated clinical
features were crowding (50.9 %), retained primary teeth (24.5%), presence of odontome
or supernumerary (8.2%) and diminutive lateral incisors (6.4%). The treatment modality
employed were orthodontic alignment (44.2 %), surgical removal (42.5 %) and monitoring
(6.2 %). Conclusion: The prevalence of impacted teeth in patients attending orthodontic
clinics in this study was found to be higher at 59.2% compared to reported international
literature. Patients presenting with this dental anomaly were generally referred at a later
age, which may prevent successful orthodontic traction and lead to prolonged treatment
duration. Hence, early detection and prompt referrals should be made for interceptive
orthodontic treatment.
