 1 Completed Form 19 of the Dental Regulations 2021
 2 Completed Personal Declaration Form
 3 *Certified True Copy of a Post-Graduate Specialist Qualification
 4 Two (2) recent colour passport-size photographs (instamatic photos are not accepted; ensure appropriate attire)
 5 *Proof of payment (e.g. NSR Certificate or official proof of NSR registration payment)

Additional Required Documents for Dental Practitioner with Previous Working Experience 

 6 *Certified copy of Registration Certificate from a dental regulatory body
7 Recent Letter of Good Standing from Dental Regulatory Body: to be sent directly to Malaysian Dental Council                                                                                                                                                                                                            

*Copy of document must be certified by Public Officer in administrative and professional posts with Grade 9 and above.

Curriculum vitae (CV) of the applicant CV must contain details of:

  • Working experience in academia or in clinical practice in the specialty (including duration)
  • Experience in undergraduate teaching and learning (including subjects and duration of teaching)
  • Experience in any postgraduate teaching in dentistry at level 7 (master) and level 8 (doctorate) MQF
  • List of publications in dental or health sciences journal

Experience in supervising research at the undergraduate or postgraduate level

 2 Evidence of specialist work experience (e.g: employment/appointment letters, logbooks, or detailed clinical/technical records)
 3 Supporting letter from current employer (if applicable)
 4 Recommendation letter from a registered specialist in the same specialty applied

Programme book containing information on the:

  • duration of training
  • mode of training
  • training module/component
  • student learning time for the programme
  • percentage of clinical training
  • curriculum of the program of study
  • scope of training, assessment, and clinical requirements
  • thesis/dissertation
  • assessment, and clinical requirements
 6 Proof that qualification is registrable as a specialist in the awarding country (Provide supporting documents from university or regulatory body).

Case portfolio/ logbook of treatment after graduation (refer to secretariat of each specialty for format for each specialty)

Submit four (4) case portfolios, which can be from during or post-specialty training. The cases submitted must reflect complexity at specialist level, or, a letter of endorsement of clinical skill as a specialist from a registered specialist in the same field in the Specialist Division of the Dental Register is accepted.

3. Application Deadlines and Evaluation:
    Application is complete only when all hardcopy documents are received at least one (1) week before the Dental Specialists’ Evaluation Committee (DSEC) scheduled meeting. Late              
    submissions  will be brought forward to the next meeting.

  4. Evaluation process:
      All complete applications will be reviewed by the Dental Specialists’ Evaluation Committee (DSEC) during its scheduled meetings. The outcome of the evaluation will be communicated        
      via  email from the Secretariat and  accompanied  by an official letter, where applicable.
