EOSINOPHILIC ULCER OF THE ORAL MUCOSA IN CHILDREN

            Introduction:  Eosinophilic ulcer (EU)  of  the oral  mucosa is a rare self-limiting oral
            condition. It is characterized by painful solitary ulcer, yellow fibrinous base with elevated
            and indurated borders. These features may resemble malignancies, traumatic ulcerations,
            deep fungal infections, tuberculosis or primary syphilis. The proposed pathogenesis is the
            ingression of microorganisms, toxins or foreign protein into the connective tissue through
            a traumatized site thus inducing a severe inflammatory response from an exaggerated
            mast cell-eosinophil reaction. Case report: A healthy 10 years old boy presented with a
            painful ulcerated lump on the lower left buccal mucosa for two weeks duration. Clinical
            examination showed an exophytic mass with ulceration present in the middle of the lesion
            measuring  20mm x 15mm. The lesion was firm, indurated and tender on  palpation.
            Another case was a healthy 8 years old girl presented with a painful and non-healing ulcer
            on the upper left posterior buccal mucosa for two months. The size was 7mm x 5mm with
            rolled  margin, indurated  and tender to palpation.  This ulcer was affecting her daily
            activity, causing reduced oral intake,  weight lost and sleep disturbance. Blood
            investigation revealed increase in lymphocytes and eosinophils count. Both cases had no
            history of trauma, lip biting or infections. No abnormalities detected from OPG. Surgical
            excisional biopsies were done for both cases and histopathology result was suggestive of
            Eosinophilic Ulcer. Upon review, the ulcers were completely healed after a month and no
            recurrence noted. Conclusion: These cases are presented to highlight the clinical features
            and management of EU. Early detection is essential to rule out other oral pathology and
            to improve patient’s quality of life.
