How to apply for APC?<br \/>\n<\/strong>The application form must be downloaded from the Nursing Division&#8217;s website which is <a href=\"https:\/\/hq.moh.gov.my\/nursing\" target=\"_blank\" rel=\"noopener\">https:\/\/hq.moh.gov.my\/nursing<\/a> &#8211; download panel- APC. The form needs to be completed by the applicant and the employer<\/p>\n<p><strong>3. What are the categories of applications and required documents?<br \/>\n<\/strong>3.1 First time application<br \/>\n3.1.1 Application form by nurse category<br \/>\n3.1.2 Official letter of application from the employer<br \/>\n3.1.3 Copy of Nurse Registration Certificate ( Fifth Schedule)<br \/>\n3.1.4 Letter of appointment from SPA for government nurses<br \/>\n3.1.5 Job offer letter for private nurses<br \/>\n3.1.6 CPD points are not required<\/p>\n<p><strong>3.2 Application for renewal of APC<\/strong><br \/>\n3.2.1 Application form by nurse category<br \/>\n3.2.2 Official letter of application from the employer<br \/>\n3.2.3 Copy of Annual Nurse Practice Certificate for the year before<br \/>\n3.2.4 The latest CPD-LB form has been verified by a supervisor<br \/>\n3.2.5 LATE FINES are imposed for renewal applications received by LJM after 30 September of the current year.<\/p>\n<p><strong>3.3 APC application for nurses who own RON for the current year or the year before.<\/strong><br \/>\n3.3.1 Application form by nurse category<br \/>\n3.3.2 Official letter of application from the employer<br \/>\n3.3.3 Job offer letter<br \/>\n3.3.4 No CPD Points are applicable<br \/>\n3.3.5 No late fines imposed<\/p>\n<p><strong>3.4 Applications for those who have had APC or RON but are inactive ( Not applying for APC or RON for more than one year )<\/strong><br \/>\n3.4.1 Application form by nurse category<br \/>\n3.4.2 Official letter of application from the employer<br \/>\n3.4.3 Job offer letter<br \/>\n3.4.4 No CPD Points are applicable<br \/>\n3.4.5 No late fines imposed<\/p>\n<p><strong>4. When is the right time to submit an application?<br \/>\n4.1 For first-time and other categories of applications<br \/>\n<\/strong>Applications should be submitted 30 days after reporting for the position.<\/p>\n<p><strong>4.2 For renewal application<br \/>\n<\/strong>Applications that have sufficient CPD credit points can start submitting applications in March each year and cannot be later than 30th September of that year. Applications received by the MNB after 30th September of the current year will be charged a <strong>LATE FINE<\/strong>.<\/p>\n<blockquote>\n<h6>Any person in the general and additional parts of the Register who wishes to carry out<br \/>\nthe practice after 31st December in a year shall, not later than 30th September of that<br \/>\nyear, make an application in the form specified in the Third Schedule and shall pay the<br \/>\nprescribed fee for obtaining a certificate to carry out such practice. (Nurse Registration<br \/>\nRegulations 1985, Subregulation 8(1)<\/h6>\n<\/blockquote>\n<p><strong>5. How much is the fee?<br \/>\n<\/strong>RM 10 &#8211; for Nurses,<br \/>\nRM 5 &#8211; for Community Nurse \/ Assistant Nurse<br \/>\nPaid in the form of a bank draft or money order to the Secretary of the Board of<br \/>\nNurses Malaysia. Please include the original Bank Draft or Money Order.<\/p>\n<p><strong>6. How much is the late fee?<br \/>\n<\/strong>A late fee of RM 10 will be charged to renewal applications received after 30 September of the current year<\/p>\n<p><strong>7. How can the document delivery process be made?<br \/>\n<\/strong>Delivery of documents to the LJM counter or delivery of documents by post<\/p>\n<p><strong>8. How long is the APC application processed?<br \/>\n<\/strong>Completed applications will be processed within six to eight weeks but the process period will be longer for applications received after 30th September.<\/p>\n<p><strong>9. What causes the application process to be late\/rejected?<br \/>\n<\/strong>9.1 No official letter of application from employer<br \/>\n9.2 Incorrect and incomplete application form<br \/>\n9.3 Supporting documents are not attached<br \/>\n9.4 Incomplete CPD-LB form, not enough CPD points and not certified by supervisor.<br \/>\n9.5 No fees, insufficient or overpayment, no late penalty payment (if applicable)<br \/>\n9.6 Bank draft or money order that has expired.<br \/>\n9.7 Original bank draft or money order is not included.<\/p>\n<p><strong>10. Can the applicant obtain a new Annual Practice Certificate if the certificate is lost?<br \/>\n<\/strong>No duplicate copies will be issued by the Malaysian Nursing Board<\/p>\n<p><strong>11. What do I need to do to get the APC serial no if my APC certificate is lost?<br \/>\n<\/strong>Applicants need to call 03-88833553 or email nursing@moh.gov.my to obtain the<br \/>\nserial number.<\/p>\n<p><strong>12. Are employers allowed to keep the original APC without providing a copy to the <\/strong><strong>employee?<br \/>\n<\/strong>The applicant must refer to the letter of agreement\/contract agreement between the applicant and the employer. The MNB is not involved in this matter.<\/p>\n<p><strong>13. What documents should be brought if the applicant wants to collect the documents <\/strong><strong>that have been completed for processing over the counter?<br \/>\n<\/strong>Individuals authorized by the employer to claim documents must submit an official letter (letter of appointment\/undertaking) from the employer together with a copy of the identity card\/passport. The Malaysian Nursing Board will not allow the collection of part of the entire document and is not responsible for any loss of documents submitted.<\/p>\n<p><strong>14. What are the responsibilities of the applicant and the employer on the APC <\/strong><strong>certificate?<br \/>\n<\/strong>14.1 The employer must keep a complete copy of the application submitted as a reference if necessary.<br \/>\n14.2 This APC certificate must be copied and stored in a safe place to avoid loss or damage<\/p>\n<p><strong>15. Can foreign nationals apply for an Annual Practice Certificate?<br \/>\n<\/strong>Foreign nationals can apply for a Temporary Practice Certificate (TPC) to work in Malaysia. Applicants must obtain prior approval to work in Malaysia from the Immigration Department of Malaysia before submitting an application to the Malaysian Nursing Board.<\/p>\n<p><strong>16. Payment must be made in the form of a Bank Draft or Money Order in the name of the Secretary of the Malaysian Nursing Board.
